Cardi-OH: A Statewide Collaborative for Better Health

Since its launch in 2017, the Ohio Cardiovascular and Diabetes Health Collaborative (Cardi-OH) has worked to improve outcomes for two of Ohio’s most significant health challenges: cardiovascular disease and diabetes. By uniting experts from Ohio’s seven medical schools, Cardi-OH created a statewide platform for sharing evidence-based strategies and driving innovation in care delivery. This collaborative effort focused on reducing disparities among Medicaid populations and promoting equitable health outcomes across the state.

Mission and Achievements

Over the years, the initiative has delivered practical resources for primary care teams, including concise clinical summaries, research updates, podcasts, webinars, and Project ECHO virtual clinics. These tools have empowered clinicians to implement best practices and foster continuous quality improvement, creating a lasting impact on patient care and provider education.

Lasting Impact

The collaborative reflects on nearly a decade of progress and partnership. Its work has strengthened statewide networks, informed policy, and improved care delivery for thousands of Ohioans. While the formal project will end June 30th, 2026, the knowledge, resources, and relationships built through Cardi-OH will continue to influence quality improvement efforts and support better health outcomes for years to come.
