A key component of the Regional QI Hub Project is the Patient and Community Engagement Collaborative. Each college of medicine participating in the Regional QI Hub project has integrated patient and family perspectives into all activities of the QI Hub program. The colleges of medicine consider a variety of strategies to gather patient voice including patients and/or family advisory groups, surveys of patients and families regarding barriers to care, opportunities to provide input into toolkits, and interventions that are patient centered and appropriate for all populations. The Patient and Community Engagement Collaborative establishes linkages to community organizations to provide enhanced access to services for patient centered care (e.g. community behavioral health providers, rental assistance/housing).
This page connects providers, patients, families, and community partners to helpful tools that support better health information and education. On this page you will find educational videos and resources developed through project work and by the colleges of medicine on the Regional QI Hub project. You will also find shared decision making guides, community resource directories, other tools, and a map of practices participating with the colleges of medicine in the Regional QI Hub project.
