The Regional Quality Improvement Hub Project is sponsored by the Ohio Department of Medicaid (ODM), and administered by the Ohio Colleges of Medicine Government Resource Center (GRC) in partnership with faculty and staff from Ohio’s Colleges of Medicine, including:
- Case Western Reserve University (CWRU) School of Medicine
- Northeast Ohio Medical University (NEOMED)
- Ohio State University College of Medicine
- Ohio University Heritage College of Osteopathic Medicine
- University of Cincinnati College of Medicine
- University of Toledo College of Medicine and Life Sciences
The Regional QI Hub also operates in close partnership with ODM representatives, external QI leaders, and Ohio’s Managed Care Organizations (MCOs).
Together, these institutions form Regional Quality Improvement (QI) Hubs that support local clinics, hospitals, and community organizations—referred to as “spoke sites”—in implementing evidence-based, scalable, and sustainable QI strategies. The focus is on chronic conditions such as hypertension and type 2 diabetes, with each college contributing unique expertise and leadership.
A key feature of the QI Hubs is their integration with Cardi-OH, a statewide cardiovascular health initiative. The QI Hub website embeds Cardi-OH resources—including podcasts, clinical presentation recordings, and educational toolkits—to enhance provider education and patient engagement. These tools help translate clinical best practices into actionable strategies that improve patient outcomes.
This project is designed to advance ODM’s population health strategy by improving care delivery for Medicaid patients with chronic conditions, reducing health disparities, and strengthening systems of care across Ohio.
