The Regional QI Hub Quality Improvement Project (QIP) seeks to support ODM’s Managed Care Population Health and Quality Strategy by targeting adults with chronic conditions (e.g. diabetes and hypertension) to ensure these conditions are screened and well managed. Population health for the Medicaid population is promoted by enabling health care delivery systems to improve performance, demonstrate value, and to reduce health disparities through interventions led by the following six of Ohio’s colleges of medicine:
- Case Western Reserve University (CWRU) School of Medicine
- Northeast Ohio Medical University (NEOMED)
- The Ohio State University College of Medicine
- Ohio University Heritage College of Osteopathic Medicine
- University of Cincinnati College of Medicine
- University of Toledo College of Medicine and Life Sciences
Each college serves as a regional hub, working closely with local clinics, hospitals, and community organizations to implement quality improvement (QI) strategies that align with the Ohio Department of Medicaid’s (ODM) population health strategy. Each Regional QI Hub is working directly with the GRC team, external QI leaders, representatives from ODM, and Ohio’s Managed Care Organizations (MCOs) to establish sustainable QI infrastructures that can be leveraged to implement and spread QI projects at the regional level to impact population health and improve health outcomes.
Project selection within the Regional QI Hub is guided by regional needs and stakeholder input with a focus on scalable interventions that build long-term QI capacity. Through this initiative, the partner medical schools are not only improving outcomes for patients today but also laying the foundation for a data-driven QI across the state of Ohio.
